Handle Above the Handle
The handles of a door are typically designed in a uniform symmetry. These handles are adjacent to each other and on an equal level. The right handle of the door in this picture is basically above the handle on the left side of the door. It is technically a handle over the handle.

This is a design mistake and can be regarded as both a “minor” and “major” one. This mistake is most prominent to the people approaching the door. It makes it difficult for a person to approach and open the door by using these two handles since the right handle is not placed properly.
